Recital Application Form

Application Procedure:

Type in applicable information on the application form below and print. Submit one form per student. Please time the piece accurately. Total playing time is limited to 7 minutes; 12 minutes for advanced repertoire. The application fee is $20.00 for student recitals and $25.00 for the holiday recital (per single entry or duets). Make a check payable to MTAC-SF and mail with the application forms to Melissa Smith (MTAC-SF Treasurer), 1504 Whipple Ave., Redwood City, CA 94062 before the due date. Telephone applications will not be accepted. Applications without payment will not be considered. Please write in your e-mail address. You will be notified when your application is received
