2006-2007 Recital Application Form
 
Application Procedure:
  • Make copies of this form. Submit one form per student.
  • Fill in applicable information clearly with CAPITAL LETTERS. .
  • Please time the piece accurately. Total playing time is limited to 7 minutes; 12 minutes for advanced repertoire.
  • The application fee is $20.00 for student recitals and $25.00 for the holiday recital (per single entry or duets). Make a check payable to MTAC-SF and mail with the application forms to a Recital Coordinator of the month before the due date. Telephone applications will not be accepted. Applications without payment will not be considered.
  • Please write in your e-mail address. You will be notified when your application is received.
Dress Code and Recital Guidelines:
  • Female performers should wear knee-length skirts or dresses, or dressy slacks with blouses. Exposed midriff (mini-skirt, slit on right side) is not permitted.
  • Male performers should wear dress slacks and dress shirt. Ties and jackets are optional.
  • Dress shoes are required for all performers. No athletic shoes or back-less sandals. Note, it is difficult to pedal with platform shoes.
  • Teachers are responsible for advising students and parents of recital etiquette. Performers should stay for the entire program and will receive the Performance Certificate at the end of the recital.
  • Flash photography is not permitted during a recital. Video cameras may be used provided they are set up in the rear of the recital hall and kept clear of the isles.
